HRMC Physicians Clinic - Chronic Care Management

If you have Medicare or are dually eligible (Medicare and Medicaid) and live with two or more chronic conditions that worsen your quality of life and put your health at risk, chronic care management (CCM) services can help. Our CCM nurse can help you navigate your health and wellness! Talk to your primary care provider about CCM.
CCM Includes:
- At least 20 minutes a month with our CCM nurse
- Personalized comprehensive health care plan to set goals for improvement
- Coordination of care between your pharmacy, doctors, specialists and follow-up appointments.
- Phone check-ins to monitor health and support progress toward goals
Eligibility
If you have Medicare, a patient at New Life Clinic or the Physicians Clinic and two or more of the following conditions or other chronic conditions, CCM can help you!
- Alzheimer's Disease
- Diabetes
- Arthritis
- Cancer
- Cardiovascular Disease
- COPD
- Hypertension or High Blood Pressure
